Preschool and adult staff members are not eligible for vaccination through this program.  Information for these populations and influenza vaccination will be coming soon.

Complete this form no later than 11:00 PM on Wednesday, September 15, 2021 if you want the student to get the flu vaccine at school. Please note information below regarding where and how the flu vaccine will be offered.

• Nearly all Olmsted County public and private schools
• Pine Island Elementary in Olmsted County; Pine Island Middle/High Schools in Goodhue County
• Kasson/Mantorville schools in Dodge County
• Preschool children do not qualify
• Only the flu shot will be available in schools

You must select which clinic (Mayo Clinic or Olmsted Medical Center) provides primary care for the student for correct billing purposes.

• If you don’t have a primary care clinic, one will be chosen for the student
• Your insurance will be billed just as if the student was seen in the regular clinic
• Vaccine will be given at no cost to you if you do not have insurance
  • An administration fee of $21.22 may be charged
  • Contact your clinic’s billing department if you cannot pay this fee

You will receive a confirmation email shortly after submitting a completed form. Forms with incorrect information may result in the student not being vaccinated at school. Please carefully answer necessary fields.
